Where Does the Term Canthal Tilt Come From?
Canthal tilt comes from ophthalmic and plastic surgery literature, where it describes the angle between the inner and outer corner of the eye, and the entire published record is small: a phrase search returns 39 papers in all of PubMed. The oldest, published in Ophthalmic Plastic and Reconstructive Surgery in 2007, carries a title that reads like a preview of the next two decades: it asked whether medial canthal tilt is a powerful cue for facial attractiveness [1]. A 2011 paper in Annals of Plastic Surgery used the measurement to analyse eyelid surgery results from photographs [2].
So the attractiveness question was attached to this angle in the clinical literature sixteen years before social media found it. What changed in 2023 was not the anatomy. It was the audience.
When Did Canthal Tilt Break Out?
Canthal tilt broke out in April 2023 and peaked in August 2023, which remains the all-time high for the term. Google Trends puts US interest at an index of 3 in March 2023, 10 in April, 37 in July and 100 in August. Before that jump the term had averaged an index near 1 for eight consecutive years. After the spike it did what most viral terms do not do: instead of returning to zero, it settled at a baseline around 7 to 8 through 2025, roughly seven times its pre-boom level, and a second wave has been building through 2026.

Why does the negative form dominate?
People search negative canthal tilt almost three times as often as positive canthal tilt, 27,100 against 9,900 a month.
